Job Description

The Support Coordinator works with participants to implement and coordinate their NDIS plans, ensuring they are connected with appropriate services and supports that align with their goals. This role involves empowering individuals, building their capacity to navigate systems, and fostering independence while ensuring quality service delivery.

Job Responsibilities

  • Assist participants in understanding and using their NDIS plan effectively.

  • Coordinate a range of supports, including informal, mainstream, and funded supports.

  • Connect participants with service providers that best meet their needs and goals.

  • Monitor progress and adjust supports as needed.

  • Develop participants’ ability to manage their own supports and make informed choices.

  • Build strong relationships with participants, families, and providers.

  • Maintain accurate records, reports, and compliance with NDIS requirements.

  • Resolve issues and barriers that may impact service delivery.

Job Requirements

  • Experience in case management, social work, community services, or disability support.

  • Strong knowledge of the NDIS and disability sector (preferred).

  • Excellent communication, interpersonal, and advocacy skills.

  • Strong organisational and problem-solving abilities.

  • Ability to work independently and manage a caseload.

  • Proficiency in record-keeping and reporting.
